Sounds similar to issues I had with my car. See the archives for posts I've made detailing what's happened. The short of it (ha, no pun intended) is that maybe there was a bad ground from the ECU, but I also replaced the cap and rotor, combo relay and it worked. I too had spark and plenty of fuel. It would crank and sputter from the fuel injected by the cold start injector but it was as if the injectors themselves weren't firing. ECU.

P.S. Of course the new ECU got it running, but it had a horrible stumble when I hit the gas under any kind of load. Replacing the old ECU got it working again. Boggle.
